CAREPATH implemented an integrated care platform focused on managing mild cognitive impairment and mild dementia. CAREPATH architecture supports multidisciplinary care teams in creating personalized treatment plans based on evidence-based guidelines, involving patients and their caregivers. The platform for healthcare professionals features modular disease pages for comprehensive care, including dedicated sections for screening complications and delivering non-pharmacological interventions such as nutrition, lifestyle, and physical exercise recommendations.

CAREPATH is an EU-funded Horizon-2020 research project that has developed an integrated, patient-centered care platform for older adults with multimorbidity, with a particular focus on people with mild cognitive impairment (MCI) or mild dementia (MD).

The platform supports screening for preclinical dementia and enables the creation of personalized treatment plans for a wide range of conditions, including frailty, sarcopenia, diabetes, heart failure, COPD, asthma, chronic kidney disease, stroke, coronary heart disease, and hypertension – always based on evidence-based guidelines and including non-pharmacological interventions.

A key objective is the active involvement of all stakeholders. Multidisciplinary care teams, patients, and family caregivers work together on care delivery. This is achieved via a set of interlinked tools:

  • The Adaptive Integrated Care Platform is a web-based application designed for healthcare professionals (HCPs), enabling them to review a patient’s medical summary retrieved from underlying Electronic Health Record (EHR) systems.
  • The Technical and Semantic Interoperability Suite enables integration with EHR systems, allowing retrieval of the most recent patient medical summary from underlying medical information systems. This data can then be processed by the AICP and CDSs. This is essential for customization of CDS suggestions to create personalized treatment plans.
  • The Patient Empowerment Platform, shares the personalized treatment plan with the patient and their informal care giver.
  • The Home and Health Monitoring Platform provides sensors and services to continuously collect real-time data for early detection of onset and changes in functioning, autonomy, and underlying cognitive and physiological functions of patients.

CAREPATH thus offers a holistic, integrated approach to improving the quality of care for elderly multimorbid patients.
